Your Membership ID is your APTA Membership number. Those with leading zeros on their membership number will need to omit the leading zeros. Step 3: Enter Your Last Name. Your Last Name is your last name with the first initial CAPITALIZED. YOU MUST CAPITALIZE YOUR FIRST INITIAL OF YOUR LAST NAME.
